THE PROGRESS NOTE
Questions to answer in each section of the progress note
Purpose:
1. What was the rehabilitation related reason for having this particular interaction with the client?
2. Why does the client (who has to be diagnosed with at mental illness in order to receive services at your agency) need your (mental health) help?
• The progress note should connect to the rehabilitation barriers (related to the client’s mental illness) and (unless you are doing something unexpected with the client) the goals that you and the client created in his/her goal setting plan.
If you are unaware about symptoms and functional deficits that could be rehabilitation barriers for the client, you should get clarification from licensed people on your team..
Action:
1. What did you and the client do? 
2. Remember:
• USE ACTION WORDS IN THE ACTION SECTION.
• Describe your observations objectively; avoid jargon
• Use quotations from client; be accurate
• Avoid value-laden words (for example instead of writing “she’s a drunk” write: “she smelled of alcohol and had difficultly walking in a straight line.”
Outcome:​

Connect your observations about the client to the purpose of the interaction.
• Focus on your Objective observations about the client.
(living situation, appearance, manner, behavior)
• If the purpose was to write a budget with the client then the outcome should be related to how the client responded to writing a budget (i.e. rather than saying “the client could not be found after I looked for him” say “the client could not be found after I looked for him and was therefore not able to complete a budget).
2. If the interaction was with your team instead of the client (discussing a plan for the client, etc) then the outcome is about how your team reacted (what they learned, what they knew, what they are going to do differently, etc)
